What kind of doctor could advise on advanced arthritis in the knee?

An orthopedic surgeon only seems interested in joint replacement, and my mom is likely too old to be a good candidate. However, she would like to maintain as much mobility as possible, and I want her pain to be managed as best as can be. But where do we go for answers?
A rheumatologist should be able to advise you about medical management of joint disease (and recommend timing of surgical intervention if warranted). There could be a significant contribution to wellbeing through the use of an exercise program or physiotherapy.

There are many ways to treat pain in the knee without having to have a knee replacement. You say your mom is too old for a knee replacement. Even if she were young enough to have one, I would not recommend it upon the advice of one doctor.
As a veteran (not old though) I injured my left knee years ago. The ortho's at the VA said I was a candidtate for knee replacement and I told them I am not. OK, getting back to your mom. Have you had more opinions on her condition? What causes her knee to hurt? If it is arthritis, that can be treated without surgery. In my opinion, since knee replacements and hip replacements became available, some surgeons are jumping on the bandwagon to make extra money for themselves.
To help your mom, wrap her knee with a warm pack that you can buy from a pharmacy. If she can tolerate anti-inflammatory meds like aspirin. That would help, also.
There are other ways to handle pain too.
